Forms

Enrollment

The Capitol Hill Cluster School urges all families to officially enroll as early as possible to ensure accurate space, class size and budget projections. Completed enrollment packages for Peabody/Watkins may be submitted at either campus. Completed enrollment packages for Stuart-Hobson must be turned in to the Stuart-Hobson front office.
Please use the forms below to enroll your student(s).

Medication in School

The Cluster teachers and staff are committed to keeping all children safe and healthy at school. Please help us by providing all the information we need about your student’s medical condition, any allergies or other conditions such as asthma, and any medications they need.

If your student has a disability or condition that requires a special accommodation, please ask the school nurse and administrators at your Cluster campus about setting up an individualized plan for your student.

Medication Forms

If your student needs medication during school hours, or has a condition that requires medication to be available, such as a food allergy, please complete the appropriate DCPS forms, which are available below.

All forms must be submitted to the school nurse. In addition, all necessary medical equipment or medication (with proper labels from the pharmacy) should also be submitted to the school nurse.

The school nurse will then work with you to make sure your student’s needs are appropriately met.

Medication and Field Trips

The school will make sure staff members who have been certified to administer medication accompany your student on field trips and during school-sponsored extracurricular activities unless you choose to accompany your student yourself.

DCPS policy requires parents to provide an extra dose of medication to be brought on the field trip on the day of the trip.

The DCPS policy on field trips, which includes information on medication and field trips, can be found here. Field Trip Policy.

If you have questions, please contact the school nurse or administrators at your student’s Cluster campus.
